During an inspection looking at part of the service

There was no manager at the service registered with the Care Quality Commission and we have written to the provider to establish their plans to have a registered manager in place. At the time of our inspection the senior care worker on duty each day was overseeing the management of the service.

During the inspection we spoke to seven staff members. We found there was not enough qualified, skilled and experience staff to meet people's needs. One staff member said, "The last few days have been a struggle, they are getting an agency in 9-5 now but before that there is only a senior and two care staff and no manager so the senior is doing the management work."

Although a system was in place for supervision, staff members had not received an appraisal and training needs for staff were not being met.

The provider did not have an effective system to regularly assess and monitor the quality of service that people received. We saw that since the last inspection the process to monitor the quality of the service had not changed.

During an inspection in response to concerns

We carried out this inspection after we received information of concern about care provided at the service.

We used different methods to help us understand the experiences of people, because most people had complex needs which meant they were not able to tell us their experiences.

We observed the relationships between staff and people appeared to be good and we found that people looked clean and well cared for.

We found that people were not always protected from risks associated with unsafe or unsuitable premises.

During an inspection looking at part of the service

The manager at the service was not registered with the Care Quality Commission and we have written to the provider to establish their plans to have a registered manager in place.

During our inspection we spoke with the manager, the provider and six members of staff.

We found that the planning and delivery of care did not always meet people's individual needs or ensure their welfare and safety.

People were not provided with suitable amounts of nutritious food in the morning to meet their needs.

We found that care records did not always contain accurate or appropriate information and documentation.

During a routine inspection

The manager at the service was not registered with the Care Quality Commission and we have written to the provider to establish their plans to have a registered manager in place.

Ten people were using the service at the time of this inspection. We spoke with four people about their experiences of the care and support they received. We also spoke with the manager and six staff.

We found that people's needs were not always assessed and care and treatment was not always planned in line with people's individual needs.

We noted that were a lot of changes in staffing on going with the service. However, observations showed that there was enough staff on duty to deliver care and the manager advised us of the recruitment that had been done to replace the permanent staff.

Staff were unable to confirm what training had been delivered and senior care workers had not received all appropriate training for their role. One senior care worker said, 'I haven't had any training for senior yet, I'm just acting up, I've been told I'll be put on my level three soon.'

The provider did not have an effective system to regularly assess and monitor the quality of service that people receive. We saw monthly audits were complete but no action plans were put in place when shortfalls were identified.

We found that care and records did not always contain accurate or appropriate information. We saw records had not been updated when people's needs had changed.

During an inspection looking at part of the service

During our inspection we looked at the care records for two people. We found care records contained accurate and appropriate information and could be accessed in a timely manner.

We saw work had been completed to improve the premises which was well maintained and designed to be accessible to all people.

During an inspection looking at part of the service

Ten people were using this service at the time of this inspection. We spoke with two of them about their experiences of the care and support they received. We also spoke with relatives of another two people, and we spoke with the manager and four staff.

We used different methods to help us understand the experiences of people, because most people had complex needs which meant they were not able to tell us their experiences.

We observed that relationships between staff and people appeared to be good and we found that people looked clean and well cared for. One person told us, 'The staff are lovely, they will do anything for you' and a visiting relative said her relative was looked after 'very well' and was 'always cheerful.'

We found that safeguarding arrangements were in place at the service and staff we spoke with had a working knowledge of safeguarding matters.

We found that people were not always protected from risks associated with unsafe and unsuitable premises.

We viewed records relating to the care and treatment of people and found they were not always accurate. Some records were not stored securely.

During an inspection in response to concerns

Fourteen people were using this service at the time of our inspection.

In most cases, we found the provider had suitable arrangements in place for obtaining, and acting in accordance with, the consent of people in relation to the care and treatment provided for them.

We spoke with three people who all told us they enjoyed the meals at Admorre House. Comments included, 'We get more than enough to eat. It's a good breakfast, you can have anything you like' and 'I can't complain about the food, I get what I want.' We found that people were protected from the risks of inadequate nutrition.

We found there were arrangements in place for the safe collection and disposal of clinical waste.

The manager at the service was not registered with the Care Quality Commission and we have written to the provider to establish their plans to have a registered manager in place.

During a routine inspection

We spoke with five people who lived at the home and two relatives to find out their opinions on the care provided. People were happy with their care and told us their care needs were being met by the staff team. One person commented that Admorre House was 'lovely'. Another said, 'it's alright here'. People enjoyed the social activities and the company of care workers.

One person told us they had thoroughly enjoyed baking shortbread with a care worker the previous day and also enjoyed the food. They said, 'you get lots of lovely trifles here'. Another person pointed to the care workers and said, 'The likes of them ones; they're good'. Another person told us they were treated well and they could have a laugh with the care workers.

A visiting relative told us that they thought the care workers 'were on the ball and dedicated to what they do'.

A number of people living at the home had dementia and were unable to tell us what they

thought about living in the home. However, people we spoke with who could communicate

their views, said they liked living at the home.

thought about living in the home. However, people we spoke with who could communicate

their views, said they liked living at the home. They said they liked the staff and the new owner. They also said they liked the food and the staff were helpful.

During an inspection in response to concerns

A number of people living at the home had dementia and were unable to tell us what they thought about living in the home. However, people we spoke with who could communicate their views, said staff were kind and helpful although they were kept very busy. They also said they were happy living in Admorre House.
