During a routine inspection

We were told patients could self-refer for treatment, for example in the case of a minor injury or headache. We saw copies of forms used to record basic details of the person concerned and the treatment provided in those cases. As the person had self-referred to receive treatment, consent was implicit. We were informed children involved with film or television productions were always accompanied by a parent, guardian or chaperone able to act on their behalf and provide informed consent to any treatment required.

We received feedback from people who commissioned services from On Set Medical Support Services (OSMSS). They told us OSMSS had an "Excellent" reputation within the film industry. One organisation told us "Their professionalism is unrivalled." Another comment was that any incidents were always dealt with "Swiftly and with the utmost care and attention." This confirmed care and treatment was planned and delivered in a way that was intended to ensure patients' safety and welfare.

We saw records of routine cleaning for the service's ambulances. This included a programme of regular deep-cleaning as well as daily and weekly scheduled cleans. There was a cleaning protocol in place for staff which set out detailed procedures to be carried out for surfaces and equipment. We spoke with ambulance staff who described the action they took to effectively address any spillages. Records for the maintenance and testing of all the service's medical equipment in use in either ambulances or the treatment centre were up to date. Out of date medication had been removed and there were satisfactory arrangements in place for the safe storage and removal of any clinical waste. We saw an appropriate. in-date clinical waste certificate was in place. This protected patients and staff from any associated risks.

Since the last inspection a recruitment process tracking system had been put in place to ensure all the required documentation and processes were complete before an appointment was made. This confirmed appropriate checks were undertaken before staff began work.

We found there was a complaints policy and procedure in place. This included contact details for the Care Quality Commission (CQC). CQC have not received any information of concern about OSMSS in the past year. All of the people we received feedback from told us they would contact OSMSS direct if they had any concerns. The provider informed us they had not received any complaints since the last inspection in March 2013.

During a routine inspection

We saw records of treatment where people self referred. Whilst the treatment was fully documented, consent to it was implied as the person had presented themselves and requested it. We records used where the patient refused treatment, transfer or examination. These set out clearly the circumstances under which the patient or guardian had declined examination, treatment or transfer to hospital or medical centre. This showed the implications of their decision had been explained to them.

We saw records for people who received treatment. They included a comprehensive record of the person's medical history. Records included an initial scene assessment and a primary and secondary patient assessment record. Various key measurements of vital signs were recorded and there were full details of initial and ongoing management of the patient. This meant the progress and extent of all treatment given was recorded.

There were effective recruitment and selection processes in place which ensured staff who provided care and medical support were suitably qualified to do so.There were also records for induction training and training in areas appropriate to the provision of medical support services.

We spoke with two commissioners of services from On Set Medical Support Services they told us they were in frequent communication with the provider and would discuss any concerns with them. They said they had not received any adverse comments about the service.
