People's personal records, including medical records, should be accurate and kept safe and confidential (outcome 21)

Meeting this standard

We checked that people who use this service

  • Their personal records including medical records are accurate, fit for purpose, held securely and remain confidential.
  • Other records required to be kept to protect their safety and well being are maintained and held securely where required.

How this check was done

We looked at the personal care or treatment records of people who use the service, carried out a visit on 6 November 2013, observed how people were being cared for and checked how people were cared for at each stage of their treatment and care. We talked with staff.

Our judgement

People were protected from the risks of unsafe or inappropriate care and treatment because accurate and appropriate records were maintained.

Reasons for our judgement

We looked at a range of records, including people’s personal records, staff records and other records relevant to the management of the service. Reviews were carried out and there was a key working system to help ensure that people’s care and support records were up to date, accurate and fit for purpose. Staff personnel records contained relevant information about their recruitment, on-going supervision and professional development. All the files were clearly indexed for ease of reference. We saw that records were kept securely and could be located promptly when needed.

There were organisational policies and procedures for the storage, retention and destruction of records, for data protection purposes. This supported the service to ensure that records were kept for the appropriate period of time and then destroyed securely. Staff signed to say they had read and understood the policy on protecting people’s confidentiality.
