During a routine inspection

People told us they were involved in their care plans and they liked the home and their rooms.

One person told us that their room was cold in the morning but the room soon warmed up in the evenings.

Another person told us the hot water in their room was not hot and their room was too cold so they had bought a heater.

We saw good practice with an open environment that had no restrictions. We noted this was achieved through appropriately managed behaviour with professional support where needed, as opposed to restrictions such as locks on food cupboards, for example.

We found the service did not have suitable arrangements to provide appropriate opportunities, encouragement and support to people who use the service in relation to promoting their community involvement.

We saw that people experienced care, treatment and support that met their needs and protected their rights.

We noted that people were not always protected against the risks associated with medicines because the provider did not have appropriate arrangements in place to manage medicines, despite a compliance action identifying the need and requiring security improvements.

We found people who use the service, staff and visitors were not always protected against the risks of unsafe or unsuitable premises.

During a routine inspection

This inspection was undertaken to look at 4 outcome areas. This was to support the previous inspection that was published in May 2012, where less than 5 outcomes (2) were looked at.

One person told us that they felt "happy and safe at Granby". Another said that the staff and service was "excellent".

Care was planned and delivered in such a way that it promoted the safety, wellbeing and independence of people using the service. We observed staff interacted with people who used the service in a professional and courteous manner. People were treated with dignity and respect.

The service did not have a system in place for managing foreseeable emergencies which were reasonably expected to arise from time to time. The manager was going to take action during our inspection to rectify this omission.

There were systems in place for receiving, administering and disposing of medication however we found that the key for the medicines cabinet was stored in an open cupboard next to the cabinet. This meant that visitors and people who used the service could access the medicine cabinet easily. We have asked the provider to make changes to how the keys are stored so that people are protected from the risks of harm associated with medicines.

We found that there were sufficient numbers of staff with the appropriate skills to meet the needs of the people who lived in the home.

During an inspection looking at part of the service

This visit was to follow up the findings from our previous visit in January 2012. The purpose of the visit was to assess if action had been taken in regards to concerns which had been highlighted at the last visit.

We did spend time talking with people living in the home during this visit, however, their feedback didn't directly relate to the outcome areas we were assessing. People indicated that they were happy living in the home. During our visit we saw that people were being supported around the home by staff in a kind and sensitive manner, in a way that promoted individual independence.

During a themed inspection looking at Learning Disability Services

There were ten people living at Granby Place at the time of our visit. Some of the people who used the service were out at a local day centre throughout the two days of the visit.

We spoke with six people who used the service. All the people we spoke with told us that they liked living in Granby Place. Comments included 'I like it here 'it feels homely' and 'I very happy and very safe here'. People spoke highly of the staff and described them as 'Lovely' and 'Helpful' with comments such as 'Staff speak to me respectfully' and 'Staff speak to us lovely'.

People told us that they were offered choices; one person said, 'I have plenty of choices, its not boring here at all, I have too many things to do' and another person told us that 'I have choices with my food, I am having my favourite today tomato soup'.

Most people said that they were happy with the activities available to them. People told us that they 'Enjoyed watching television,' 'Going for walks,' 'Listening to the radio,' 'Going to the hairdresser' and 'Going to the Day Services'. However, one person said that some activities had been 'Cut back' and that there were 'Not enough staff' and said that this worried them.

Everyone we spoke to said that they felt 'safe' living in Granby Place. Some of the people we spoke with said that they had been told about different types of abuse and how to report it. People told us that they had not seen any abuse in the home and said that if they were worried they would speak to staff.

We spoke with two relatives of people using the service who told us that they were happy with the service provided and praised staff for their care and support. Comments included 'name is well looked after and their choices are well supported' whist another relative told us that 'I am exceptionally happy with the care name receives at Granby Place; and am hugely impressed by the patience and compassion that all staff show to name.'

The relatives we spoke with told us that they were kept informed about their relative's progress. One relative stated, 'We are kept up to date of any changes or issues and I am invited to name's care reviews regularly'. All the relatives we spoke with told us they felt their relative was safe at the service and knew who to speak to if they were unhappy with aspects of their relative's care or support.
