We checked that people who use this service

  • Receive safe and coordinated care, treatment and support where more than one provider is involved, or they are moved between services.

How this check was done

Our judgement

The service understands the importance of working with other providers of care for individuals and has in place systems to ensure this happens. On the basis of the evidence provided and the views of people using the service we found the service to be compliant with this outcome.

User experience

We spoke to one user of service who gave us an example of how his care was coordinated between the provider and the district nurse.

Other evidence

The provider told us that they share information with other professionals when required in a confidential manner. Care is taken that all such information sharing processes comply with the relevant regulation. We spoke to one district nurse who said the service worked well with her. When they are issues or concerns, these are resolved promptly by the organisation. We spoke to a staff who shared with us an example of how a multidisciplinary meetings are held to establish the needs of the people who use the service.
