During an inspection looking at part of the service

We inspected the service in September 2012 and we found that there were no systems in place to assess and manage risks including the prevention of the risk and spread of infection and environmental risks that might pose a hazard to people who used the service or to staff. The service sent us an action plan telling us how they would ensure the above standards were met

We also referred the matters from this inspection to East Riding of Yorkshire Council's environmental health department and the Fire Safety Assessor for Humberside Fire and Rescue Service and asked them to confirm the home met their specific requirements.

We revisited the home and walked round the premises to see if the required improvements to our compliance actions had been made. We talked with staff and reviewed the action plan. We saw that both agencies had visited the home and that actions had been required. We saw that the majority of the actions had been completed and plans were in place to complete the rest.

When we visited the service in September 2012 people who used the service were satisfied with the care they received and their homely environment. We chatted briefly with people during this visit but their comments to us did not relate to the outcomes we were inspecting.

During a routine inspection

People spoke positively about the care they received at Northgate House. They told us they felt the staff really went out of their way to make sure they were well looked after.

We observed that members of care staff were friendly and courteous when they spoke to people who used the service and included them in general conversations. Visiting healthcare professionals were also positive about the care staff and said they found they were helpful and willing.

However we found the home was lacking clear management and leadership on a day to day basis and we identified improvements were needed in several areas. There were no systems in place to assess and manage risks including the prevention of the risk and spread of infection and environmental risks that might pose a hazard to people who used the service or to staff.

During an inspection looking at part of the service

We had not intended speaking with people during this follow up inspection. However, during the visit two people told us they were happy living in the home, they liked the meals provided and staff looked after them well.

During a routine inspection

People told us they liked the home and that it was always kept clean and tidy.

People told us about the decisions they could make in relation to day to day living. Generally this was fine but they said they missed the activities that no longer took place and they could easily get bored.

They liked the meals and said there were alternatives to the menu, "he is well fed - it always looks and smells nice" and "yes it's ok ' you get what you want to eat".

People told us they were happy with their care and that they were well looked after. They saw their GP and other health professionals when needed.

People knew how to complain and said they would speak to the manager about any concerns.
