During an inspection looking at part of the service

During this short responsive inspection we spent time on the dementia unit where we observed people in their own environment. We had some conversations with people and they told us:

"It is grand...like a feather bed!"

"This is my home...nothing wrong here."

"I have friends here and we have an easy time together."

We went to check that the written plans of care were more robust for people with dementia. We had found at the last inspection that the plans did not help staff support people appropriately. At this visit we saw that all the plans in all areas of the home had been reviewed.

We looked at all of the plans for the people who lived on the special dementia care unit. We saw that all of the plans for people with dementia were much more detailed. We saw, for example, that where people had problems controlling their emotions and behaviour the team had started to write more specific guidance for the whole team to follow.

Staff told us that there had been team meetings where they were helped to look at the way they helped individuals. There had been some updates to staff training. They also told us that they had asked for support from mental health professionals.

We judged that these developments meant that the service was now compliant. We asked the provider to continue to support the team to develop strategies for people with dementia.

During a routine inspection

We spent time with some of the people who had made Castlemount their home. They told us that they were happy with the care and services provided. These are some of the things people said:

"I have settled well...everything is very good..."

"I am happy here...everyone is very nice."

"I like it here...the staff are really good, both day and night and I get everything I need.

We judged that the written plans of care were of a reasonable standard for most of the people in the home but some plans for people with more complex needs were not detailed enough.

The food provided was of a good standard and people told us:

"The food is very nice and I have no complaints."

We checked on medication held on behalf of people and this was being managed correctly.

We saw that the home was neat and tidy on the day of our visit. The home had upgraded the dementia care unit to help people who might find it difficult to orientate themselves.

We judged that there were generally enough staff to give people the care and services they needed.

We looked at a range of records and found them to be up to date and to provide the information necessary to operate the service.

During a routine inspection

We found that the home involved people and their carers in all aspects of planning their treatment and support. People told us they were fully aware of their care plan and said they had been involved in stating their wishes. One person told us; 'My daughters come in every day but the manager and supervisors also come for a chat to see how things are.' We examined care plans and found that people's privacy and dignity was recognised and staff sought their views in a number of different ways. Staff demonstrated that they understood the importance of people being given good quality and timely information about every aspect of their care, treatment and support.

When we spoke with people living at Castlemount they told us they were happy living there and they were pleased with their own rooms, they also said they liked being able to decorate their rooms as they pleased and to have their personal belongings in their bedrooms. On the day of the inspection the home was found to be clean and tidy. However we found that some areas of the home were looking slightly run down, with some signs of heavy wear and tear on walls, skirting boards and flooring.

Systems were in place which ensured the training and development needs of staff were assessed at the start of their employment and they were provided with on-going suitable training for their role.

This was a review to look at arrangements to meet fire safety, and did not include a visit to the service.

The previous inspection carried out on the 28 June 2011 has comments from people living in the home and other professionals about the care and service.

During a routine inspection

People said were very pleased with the care and support they receive at the home.

These quotes sum up the feelings:

'I cannot rate the home highly enough- but I would say it is excellent'

'The staff always have time, and make time for you'

'I have lived here for 10 years, this is my home and I am very happy with the care'

'I trust the girls to look after my care'

The home involves people and their carers in all aspects of planning their treatment and support, which may affect their lifestyle and quality of life. When we spoke to people they were fully aware of their care plan and said they had been involved in stating their wishes.

The manager says she likes to promote an open culture and minimise any formal need for people to complain. People we spoke to were in agreement with this and said they feel very comfortable with the staff and could raise any issues with them, and know that these would be listened to and acted upon.

People spoken to said 'You can do as you please here'.

On our visit we saw people choosing how to spend their time, some were engaged in interests, such as crosswords, reading and knitting. Others told us they liked to spend time in their own rooms while others people chose to sit in the communal lounges. There was a calm and relaxed atmosphere and people were seen having their care needs attended to in a timely and supportive manner. People told us that there were enough staff to meet their needs.

On the day of the visit the home was found to be clean and tidy. And people told us that their rooms were regularly cleaned and bedding changed.

One person said 'It's spotless and I like the way we have fresh flowers'
