During a routine inspection

During our visit to Horncastle House, there were 27 people living there and we spoke with six of them. We spoke with the manager, the area manager, visitors, and staff. We found that people were receiving a good level of care and support from the provider. People we spoke with told us: 'am well looked after', 'excellent food and nothing to complain', 'staffs are very kind and supportive', 'dad loves it here', 'X the manager and her team are excellent'.

We read through personal care files and found that thorough assessments were done before admission and people or their next of kin were involved in their care planning. There were risks assessments in place and we found that they were reviewed regularly. We saw evidence of people's privacy and dignity were being respected. We found that people were receiving care appropriate to their needs and their welfare well supported by the team of staff.

We found that there was a robust recruitment procedure in place and staff had the required skills to carry out their role effectively. We noted that staff were conversant with the local safeguarding policy in place. People we spoke with told us that if they had any concern they would raise it with the manager and they were aware of the safeguarding procedure in place.

We found that accurate and appropriate records were kept about the management and delivery of the service. Records were fit for purpose, securely stored and accessible to only designated members of staff.

During a routine inspection

We spoke with eight people who used the service and one relative. People told us they were happy living at Horncastle House. One person told us, "I feel comfortable" and felt the it was "Like home." Others told us the service was "Quite nice" and "Perfect." People spoke highly of the staff. One person said they "Get on very well with staff. They are like companions.' One relative said the staff were "Very good." People told us their consent was obtained and they had many choices in their daily life. People's care plans were reviewed regularly which ensured that they were accurate and fit for purpose.

People said their environment was clean and tidy. They told us that the home and their rooms were cleaned daily and that the home was "A tidy place.' We observed that staff maintained good infection control practices.

We found that there were sufficient staff to meet the needs of people who lived at the home. People's needs were responded to in a timely manner and staffing levels maintained people's safety.

We found that the service had systems in place for obtaining and responding to complaints and concerns. People were given information about how to raise a complaint and had confidence in the manager to respond appropriately. People said that staff listened to them and took action where needed.

During a routine inspection

Some of the people that use the service at Horncastle House have dementia, therefore not everyone was able to tell us about their experiences. To help us to understand the experiences people have, we used our Short Observational Framework for Inspection (SOFI) tool. The SOFI tool allows us to spend time watching what is going on in a service and helps us to record how people spend their time, the type of support they get and whether they have positive experiences.

We spent 45 minutes watching when people who used services had their dinner and found that overall people had positive experiences. The staff supporting them knew what support they needed and they respected their wishes if they wanted to manage on their own. We saw that people were offered choices. The support that we saw being given to people matched what their care plan said they needed.
