People's personal records, including medical records, should be accurate and kept safe and confidential (outcome 21)

Meeting this standard

We checked that people who use this service

  • Their personal records including medical records are accurate, fit for purpose, held securely and remain confidential.
  • Other records required to be kept to protect their safety and well being are maintained and held securely where required.

How this check was done

We carried out a visit on 19 April 2013, checked how people were cared for at each stage of their treatment and care, talked with people who use the service and talked with carers and / or family members. We talked with staff.

Our judgement

The provider held an accurate record in respect of each service user which included appropriate information and documents in relation to the care an treatment provided to each service user.

Reasons for our judgement

At our last inspection in September 2012 we found the provider was not meeting this standard. We found people were not fully protected from the risk of unsafe or inappropriate care and treatment because some records relating to care plans, risk assessments and risk management plans were not accurate and up to date.

All of the support plans we looked at included individual risk assessments and support plans for supporting people within their home environment. They included people's medication, risk of falls, moving and handling and mobility. These had been reviewed to accommodate the changing needs of each person. Copies of which were held at the main office but also at peoples homes. We saw evidence of these risks being reviewed and updated on peoples care plans.
