During an inspection looking at part of the service

When we inspected Weald Hall on 6 January 2014 we found that four people's personal files contained incomplete assessments and that care plans and reviews were not appropriately recorded. This meant that people who used the service were not protected against the risks of unsafe or inappropriate care and treatment arising from a lack of proper information about them. The provider did not maintain accurate records in respect of people who used the service and was therefore non compliant with Outcome 21, regulations 20; Records. We asked the provider to send us an action plan telling us what actions they would take to become compliant with Outcome 21, regulation 20; Records. The provider sent us an action plan which said they would be compliant with this outcome by 31 March 2014.

We carried out a follow-up inspection of Weald Hall on 2 March 2014 to check that the action plan had been implemented. At this inspection we found that the provider had achieved compliance with the Regulation 20 of the Health and Social care Act 2008 (Regulated Activities) regulations 2010.

We found that a new format of templates had been adopted and new recording systems were in place. We saw that all records were appropriately updated. A member of staff told us, "We ensure all information is appropriately recorded, the new system makes recording much easier to maintain".

During a routine inspection

We spoke with the home manager, the deputy manager, the administrator, five members of care staff including two senior care workers. We also spoke with five people who lived in the service and one of their relatives. We looked at people's care plans, personnel files, the service's policies and procedures. We also checked how records relevant to the service were maintained, stored and disposed of.

We found that people were provided with a clear statement relevant to the fees, terms and conditions prior to the start of the service. People were given time to consider whether they wished to proceed with their residency, care, treatment and support. One relative of a person who lived in Weald Hall told us, "The agreement of residency and the fees were clear from the beginning".

We looked at ten sets of records for people who used the service and observed that comprehensive care plans and risk assessments reflected people's needs and that care and treatment were delivered promptly and appropriately. People were appropriately referred to healthcare professionals when needed. People participated in a wide range of activities. A person who used the service told us, "I am never bored, there is plenty to do if I want to join". A relative told us, "We had a look at other homes and found this one had a homely cosy atmosphere with attentive staff and activities, it felt right".

We found that the staff were trained and knowledgeable in the safeguarding of vulnerable adults. A member of staff told us, "I remember the training and I would not hesitate to contact our manager or social services if I suspected any abuse was taking place".

We found that some of people's files were incomplete due to missed recordings and updates. We found that some records could not be located promptly by staff due to an overload of documents which was contained in broken folders. We therefore found the provider non-compliant with Regulation 20 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010 and asked for this to be remedied.

During a routine inspection

During our inspection we spoke to six people who used the service, one relative, four members of staff and the manager.

We found that people's privacy, dignity and independence were respected and that they were involved in the way their care and treatment were provided and delivered. One member of staff told us 'We always explain things to people and we always respect their choices'.

We found that the service offered a wide range of optional activities for people who lived in the service. One person who used the service told us "I am never bored here".

We saw that the service sought consent from people for all aspects of their care and treatment. We found that comprehensive care plans and risk assessments reflected people's needs and that care and treatment were delivered promptly and appropriately.

We found that the meals provided were of a high standard and that people's nutritional needs were met. One person told us "I like the food here it is always tasty and hot". Another person told us "The food is really excellent, I look forward to it every time".

We saw that members of staff were appropriately supported and were enabled to deliver care and treatment safely to people who used the service. We found that they received appropriate induction, training, supervision and appraisal as well as opportunities for professional development. A member of staff told us "I am happy with what we are achieving here in Weald Hall, I am proud to be working here".
