During a routine inspection

Concerns had been raised with us therefore we specifically looked at care planning documentation, medicine management and complaints management. We looked at the number of people with end of life care needs who had died in the home. We have asked the provider to follow up reports that night staff were sleeping on duty.

We have asked the provider to make improvements with people's care plans and the way they were reviewed and updated. Improvements were required with the assessment of risks and monitoring of healthcare needs. Although there was a formal complaints procedure not all complaints made by families were logged as complaints.

People in the home had dementia and were unable to tell us their views about the way they were looked after. We completed a short observation framework inspection (SOFI) and spent a period of time observing the interactions of staff with a group of people. On the whole we observed positive interactions; the staff were attentive to people's individual needs and genuinely cared for them. Three examples where the interaction could have been better were discussed with managers.

We discussed procedures in place for administering medicines to people who refused to take their tablets. There were satisfactory safeguards in place to ensure that medicines were administered safely, or marked as refused.

Recruitment and selection of staff ensured that appropriate checks were undertaken before they began work in the home.

During a routine inspection

Due to the very complex needs of resident's we were not able to speak to people who lived at The Grange. However we were able to observe during our visit that visitors were welcomed and staff were always friendly and polite. We saw that staff were engaging and gentle in their approach to residents for example, during lunchtime staff sat with residents who needed support. We observed staff supporting people during an activity session, staff were engaging and encouraging and spoke to people addressing them by their name.

We found that care plans were person centred and that individual needs were recorded daily. Staff told us that they worked closely with other professionals when delivering care and support. For example, staff worked with GPs and hospital departments. Staff told us that personalisation of care was important as it meant that resident's were seen as individuals first.

People were safeguarded against the potential of abuse and there were arrangements in place to guide and support staff for safeguarding vulnerable people.

Staff told us they were supported by the management team and that they received regular supervision and annual appraisal.

Staff informed us that they received regular training and from time to time were able to undertake qualifications regarding their roles.

The provider had a range of systems in place to monitor the quality of its service delivery and to act where improvements were required.

During an inspection in response to concerns

The vast majority of people who live at The Grange Care Centre have dementia; therefore we did not ask them direct questions about their care.

Before this inspection we received information which gave us some concerns about the service provided to people particularly, in relation to how they were moved and handled by staff.
