During a routine inspection

This inspection was carried out in order to review two compliance actions set at our last inspection in September 2013. We had concerns that people who lived at Fistral House did not have their views and experiences taken into account in the way the service was provided and delivered in relation to their care. We also had concerns the provider did not have appropriate arrangements in place to manage medicines.

We spoke with four people who lived at Fistral House and one visitor. Their comments included "I am very happy here" and "they do very well". People were positive in their comments regarding the care and support they received.

We spoke with two staff, the registered manager and the deputy manager and reviewed the daily records for three people.

We found people's views and experiences were taken into consideration in the way the home was run, and people were provided with choice, although we did not see documentation to support this.

We saw there were arrangements in place to protect people from the risks associated with medicines.

During a routine inspection

We spoke with four people who lived at Fistral House, their comments included 'it's like home from home', and 'the staff are very sweet'. We spoke with five staff who work at Fistral, the registered manager, and the deputy manager. Some of their comments included 'we are like one big family here' and 'we are a team'. We spoke with healthcare professionals who visit the home regularly and they did not raise any concerns regarding the care provided at Fistral House.

We found peoples' views and experiences had not been taken into account in the way the service was provided and delivered in relation to their care. People were not always treated with respect and dignity.

People were protected from the risks of inadequate nutrition and dehydration.

People were not protected against the risks associated with medicines because the provider did not have appropriate arrangements in place to manage medicines.

We saw there were appropriate checks undertaken before staff commenced working for the service.

People were cared for by staff who were properly supported to deliver care and treatment safely and to an appropriate standard.

During a routine inspection

At the time of the inspection eleven people lived in the home. On the day of the inspection, we spoke with nine people who lived at Fistral House. People were all positive about the care and support they received. The majority of people said the staff were supportive and caring. Typical comments included 'the home is lovely, I could not have wished for a better home,' and 'I am very comfortable here, there is plenty to eat and I am well looked after.'

People said the food was to a good standard. People said they were encouraged to make choices and there was a relaxed atmosphere.

When we inspected the home was clean and odour free. The home was well maintained and well furnished.

Staff recruitment and staffing levels were to a good standard. Staff had received some training but improvements were required in this area as staff had either not received some training which was required by law and/ or updates were required. However staff were observed working professionally and doing their best to meet the needs of the people that lived in the home. Quality assurance systems were adequate.

During an inspection in response to concerns

We saw people's privacy and dignity being respected and staff being helpful. There were no issues raised by people using the service or staff. People who were able to were moving freely around the home.

People told us that the staff were 'lovely' and 'excellent' and 'patient' with 'nothing being to much bother' for them. They said they had enough to do and could have visitors when they wanted.

We saw that the routines being observed during the inspection showed that people have choices about where they spend their time. People we spoke with confirmed that they chose how and where to spend their time, and that they could do what they liked.

We observed staff and were impressed with the patience and care shown to people using the service. People told us they were happy with the care they received at Fistral House Residential Home.

People told us that they liked their rooms and the fact they could bring personal items with them when they moved in.

People told us that they were happy with the equipment provided to them and that the staff were competent in its use.

All of the staff we spoke to said they were all happy working at Fistral House Residential Home
