During a routine inspection

This inspection was carried out in order to review areas that required improvement compliance actions made following the previous inspection completed on 12 June 2013. During this inspection we found improvements had been made with some areas still to address. There were no service users at home during our inspection but this did not impact on our ability to assess progress with compliance actions. We did speak with staff working at the home and the manager and owners.

Systems were now in place to ensure the safe recruitment and induction of staff.

Staff had received a programme of training which had brought them up to date with regard to mandatory health and safety training. Further training was planned to enhance staffs' knowledge and skills with regard to meeting the needs of the people living at the service.

The provider had begun to implement systems to monitor and assess the quality of the service but this still needed further improvement.

Records required were up to date and accurate. Care plans were comprehensive and provided staff with sufficient information to provide support to people according to their needs.

During an inspection looking at part of the service

This inspection was carried out to follow up on compliance actions made at the previous inspections of 29 April 2013, 4 December 2012 and 24 September 2012 . We were supported on this inspection by an expert-by-experience. This is a person who has personal experience of using or caring for someone who uses this type of care service. We were accompanied by a specialist advisor.

Since the previous inspection the registered manager had made some improvements, particularly in the deployment of staff, arrangements for the storage and administration of medication, areas of the environment and safeguarding training for staff. However, there are still areas which need addressing for which the registered manager has provided an action plan. We will continue to monitor the progress of the action plan by asking the registered manager to submit documentary evidence and by carrying out responsive review visits to the home.

We spoke to people living at the home; they all said they were well cared for by staff, they had good relationships with all the staff and were respected.

During an inspection looking at part of the service

This inspection was carried out to follow up on compliance actions made at the previous inspections of 24 September 2012 and 4 December 2012. Partner agencies had shared with us their concerns and action they had asked the service to make. We have used some of this information to help us inform our judgements.

People told us they were happy with the support they received; they told us that they were included in making decisions about their daily lives.

We found that since the last inspection the provider had not sufficiently addressed many areas of concern. These areas included people's choices, health and welfare and also staff recruitment, training and practice. We found that people's care plan documents did not provide sufficient information to demonstrate that care and support was being provided to meet people's needs. We also found that staff were not safely recruited and staff had not received any up to date training.

We found that the home was not being well managed. As a result documentary paperwork was not available and the manager could not evidence for us that the home was managing risks to the health and safety and wellbeing of people living at the home effectively.

During a routine inspection

We decided to bring forward this scheduled inspection because of concerns raised by the Local Authority Commissioning and Contracting team and the PCT ( Primary care Trust) safeguarding team. They were concerned about how the home was being managed, care plans, management of some safeguarding adults issues and a lack of staff training.

We carried out this inspection over two days because we wanted to speak to the manager who was off sick on the first day, and we wanted to talk to people who use the service who were out at sheltered employment or day services on the first day. The manager was still off work with ill health on the second day.

We talked to seven people who live at the home. They told us that they were happy living at the home and that staff were kind and friendly. They told us about holidays and activities which they had enjoyed.

We asked two people about sharing rooms and they both told us that they were happy with these arrangements. They told us that they could talk to staff if they were unhappy about anything.

During an inspection in response to concerns

We visited this service because we had concerns that staff were not recruited to the home in a safe way. These concerns were raised by North Yorkshire County Council. We visited the service to look at the recruitment practices at the home.

We found the recruitment systems in place at the home were not effective and people were being placed at unnecessary risk of harm because staff were not recruited to the home in a safe way. We have asked the provider to take action to improve the way they recruited staff to the home.

During an inspection looking at part of the service

People told us that they took part in day-to-day domestic tasks in the home. They also told us that they enjoyed a good range of activities and were well supported by the staff at the home.
