During an inspection looking at part of the service

We carried out this responsive review to check on compliance of outcome 9 only, the management of medicines. On our previous inspection of this service on 25 May and 9 June 2011 where we found that the service was not compliant with outcome 9. We gave them information about why they were not compliant and a timescale by which they needed to comply by. This review specifically looked at what they have done to reach compliance.

We did not on this occasion talk to any service users. We are aware that the detox part of the service is currently not in operation and therefore there are less medication on site.

We spoke to staff and check the medications and records during this visit.

During an inspection in response to concerns

We completed this responsive review following an alerter raising some key concerns about three main areas, staffing levels, management of medications and some staff allegedly engaging in sexual relationships with service users.

This review included two visits to the service on 25 May and 9 June 2011. During this time we spoke to five people currently at the service and all of the staff working on those days.

One person told us they had only been there for a short time for detox, but had found all the staff 'very helpful and had listened to concerns raised about some issues.' They said that staff working in the detox unit 'know their stuff' and said they felt positive about getting to the next stage.

Another person told us that they had been 'very well supported through a very difficult time' They felt that staff at Lampton Court had understood their complex needs.. .'

One person said that they thought the food was 'not good' and that improvements should be made in respect of the gym equipment but when asked about their treatment and experience they said that overall it had been' good'. One person said some staff were more helpful than others and another told us 'if you are willing to take part in the group work sessions and work on your own journal and be honest then you can get a lot out of this programme. Some people are less committed and don't put the work in themselves.'

We found that good systems were in place for ensuring a pre admission assessment was completed, but that for one person there was a lack of detail in the plan of care and potential risks had not been fully identified.

We checked records and management of medications and will be looking at further actions in respect of this.

We saw that staffing levels were sufficient for the regulated activity but that there had been a higher than average turnover of staff and we have asked for an improvement plan to address this.

We found that there were two occasions where staff had engaged in personal relationships with ex service users and we have set a compliance action in respect of this.

During an inspection in response to concerns

People who are currently living at Lampton Court told us that they thought that they were being supported to go through a rehabilitation of their dependency from drugs and/or alcohol. Overall comments we received from speaking to a total of five people were positive and included

''Staff are all sound here''

''Some staff are better than others, but mostly they are good.''

'We can talk to our counsellor or the manager and staff if we have any complaints''

''Yes I feel they do listen to us, I have a problem, but not with this service and the manager is trying to help me with this''

''We have a resident representative who meets with the manager weekly''

'We are taken to the doctor if needed and always on time'

'We are happy with how medicines are looked after'

Some people told us that they can see a different doctor at the local surgery if they don't want to see the doctor who visits the service.
