We carried out a review of the data available to us about Mansfield Manor Care Home on 6 July 2023. We have not found evidence that we need to carry out an inspection or reassess our rating at this stage.

During an inspection looking at part of the service

About the service: Mansfield Manor is a care home that provides personal care for up to 38 people. At the time of the inspection 36 people lived at the home, most of these people were older people, some of whom were living with dementia.

The providers COVID-19 policies were not up to date and had not reflected the latest guidance. This meant we could not be assured by all the aspects of managing the infection control needs. The domestic staff had cleaning schedules to ensure the home was routinely cleaned. This included steamers for fabric coverings. However, touch cleaning was not continued after the domestic staff had completed their hours. The provider reviewed this aspect of cleaning and made adjustments to the cleaning schedule to reflect on going touch surface cleaning.

On arrival visitors had not been routinely asked to complete a questionnaire to support track and trace, the provider implemented this procedure along with a temperature check.

We found the following examples of good practice.

¿ We found care home testing was in place for both people using the service and the staff. Testing has been increased since the COVID-19 outbreak. Daily temperatures were being taken; however, these were increased to twice daily to meet government guidance.

¿ There were sufficient supplies of personal protective equipment (PPE), which were placed around the home for easy access.

¿ There were currently no visitors within the home, however technology was used to support calls to family thought this method or the telephone. The home had a visitor’s ‘pod’ the use of which will be resumed as soon as possible.

¿ Staff were encouraged to change into their uniform on site, to reduce the transmission of COVID-19. The provider was reviewing areas of the home to establish a larger space and easier access.

¿ Staff had received training in wearing their PPE and the importance of changing between each person. The manager told us they complete regular competency checks to ensure staff continued to be compliant in this area.

During a routine inspection

About the service: Mansfield Manor is a care home that provides personal care for up to 38 people. At the time of the inspection 34 people lived at the home, most of these people were older people, some of whom were living with dementia.

People’s experience of using this service: Overall, people had a positive experience of living at Mansfield Manor. Some improvements were required to safely manage risk and to ensure people received their medicines as required. Changes had been made after incidents to reduce the risk of the same thing happening again. People told us they felt safe and staff, and the manager, had a good understanding of safeguarding adults’ referrals to the local authority safeguarding team as required. The home was clean and hygienic, and people were protected from environmental risks.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice. Some staff did not have training in key areas, training had been planned to address this. Despite this, we found staff to be skilled and competent and staff told us they felt supported in their roles. People had a choice about what they ate and had enough food and drink. People had access to healthcare when they needed it, and advice was sought from specialist health professionals.

People were supported by staff who were kind and caring, staff knew people well and people were involved in making decisions about their care. People were treated with dignity and respect and their right to privacy was upheld.

The service was flexible to meet people’s needs. Staff had a good understanding of how to support people and people chose how they spent their time. Further work was needed to ensure care records were used effectively by staff. There were systems in place to respond to complaints and concerns. People were provided with caring and compassionate support at the end of their lives.

The home was well led. There were effective systems to ensure the safety and quality of the service. The management team had already identified most issues prior to our inspection and had plans to address the concerns. During and after our inspection the management team were responsive to our feedback and took swift action based upon this. People and staff were involved in the running of the home. The home worked in partnership with other professionals to ensure good practice.

Why we inspected: This was a planned inspection based on the rating at the last inspection.

During a routine inspection

We inspected the service on 7 September 2016. The inspection was unannounced. Mansfield Manor Care Home provides support and nursing care for up to 38 older people, some of who live with a dementia related illness. On the day of our inspection 34 people were using the service.

The service had a registered manager in place at the time of our inspection. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons.’ Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

People were supported by staff who knew how to recognise abuse and how to respond to concerns. Risks in relation to people’s daily life were assessed and planned for to protect them from harm.

People were supported by enough staff to ensure they received care and support when they needed it. Medicines were managed safely and people received their medicines as prescribed.

People were supported by staff who had the knowledge and skills to provide safe and appropriate care and support. People were supported to make decisions and staff knew how to act if people did not have the capacity to make decisions.

People were supported to maintain their nutrition and staff were monitoring and responding to people’s health conditions. People were supported to enjoy a social life and to have their spiritual needs met.

People lived in a service where staff listened to them and treated them with compassion and patience. People’s emotional needs were recognised and responded to and when people reached the end of their life staff went the extra mile to ensure their wishes were met.

People were involved in giving their views on how the service was run and there were systems in place to monitor and improve the quality of the service provided.

During a routine inspection

We used a number of different methods to help us understand the experiences of people using the service, because they had complex needs which meant they were not all able to tell us their experiences. We looked at records. We spoke with six people using the service and one visitor. We visited people in the main lounge and bedrooms. We spoke with the provider, manager and three staff. We observed how staff interacted and supported people using the service.

People using the service told us they were happy and felt safe at Mansfield Manor, comments included, 'I am happy here, they do their best for you.' Also, 'I cannot complain, the food is lovely and the staff are nice.'

A visiting relative told us, 'I think it's wonderful here, I have no concerns at all.'

We found that people using the service were supported to receive care and treatment safely. We found the provider had made improvements to the quality assurance and care planning systems since our last inspection.

During a routine inspection

We used a number of different methods to help us understand the experiences of people using the service, because they had complex needs which meant they were not all able to tell us their experiences. We looked at records. We spoke with four people using the service and two visitors. We visited people in the main lounge and bedrooms. We spoke with the provider, manager and three staff.

We found that people who used the service were satisfied with the care they received and as far as possible they had been involved in planning their care and treatment. We found that care and treatment was not planned or delivered in a way that ensured people's health, safety and welfare and the quality of the care planning was not being assessed by the provider.

There was an active social calendar and people who used the service were supported to remain as independent as possible. Comments included, 'I go out regularly with my family and visit the local church.'

Two visiting relatives told us that they felt confident in the services being provided at the home.

During an inspection in response to concerns

We inspected this service because we had concerns that we had not visited since 2008.

People who used the service felt that the staff were very caring and did their best to look after them well. One person said, 'I feel safe here, they do the best they can for me. Some of the staff are very caring, I complained once and they acted on that straight away.'

A relative of a person who was unable to tell us about their experience told us, 'I like it here, I would not leave my relative anywhere else. They keep everywhere clean, the food is excellent and the staff are good.'
