During an inspection looking at part of the service

This was a follow up inspection of Orsett House in relation to issues which had been identified on our previous inspection in December 2012. During the previous inspection we found that people who used the service were not receiving care appropriate to their individual needs. We found issues with the management handling and dispensing of medicines and we found that staff were not being supported to undertake appropriate training.

As a result of our findings the home provided us with an action plan outlining how they would address the issues. During this inspection we saw that the action plan had been implemented and this had resulted in great improvements in all the areas of concern.

We reviewed care records and other documents to ensure that people who used the service received care appropriate to their needs.

People told us that they were happy with the care and support they received. One person who used the service told us, "I think I'm very lucky to be here'.

We saw that medication was dealt with correctly. Procedures were followed and records made which kept people safe.

Training, support and supervision enabled staff to deliver care to an appropriate standard. A member of staff told us, "Things have really improved over the last few months'.

During a routine inspection

We saw that the home had systems in place to gain consent from the people who used the services or their relatives. People we spoke with told us that they were always involved in any decision making.

The plans of care and risk assessments did not reflect all of the needs of the people using the service.

We spoke with relatives of people who used the service and staff. People told us that they were happy with the care that their relatives received and stated, "It's a very good home", "I am extremely pleased" and "I am impressed with the compassion that is shown by the staff".

We saw that medication was stored correctly and weekly audits were undertaken, but concerns were identified with regards to the administering and recording of medication.

We saw that staff had been recruited in an appropriate way and checks had been undertaken to ensure that they were suitable to provide care for vulnerable people.

Staff we spoke with told us that they felt supported by the provider through induction and regular supervisions. We spoke to staff and viewed training records, which identified a shortfall in training for dementia care, infection, control, abuse and deprivation of liberty.

We saw the home had systems in place to record and investigate complaints about the service. People we spoke with told us that when they had complained appropriate action had been undertaken to rectify the problem by the registered manager.

During an inspection looking at part of the service

We carried out this review to check on the care and welfare of people using this service, our records showed that there had been no recent inspection of this service. There were 49 people living at Orsett House when we visited on 15 December 2011. The visit was unannounced which means the provider and the staff did not know we were coming.

The home felt warm and very welcoming. We saw that communal areas had been decorated for Christmas and there was a festive ambiance around the home. We spoke with people living at the home, visiting family and friends and staff members. People living at the home told us 'I am very happy here; the staff look after me well and respect my privacy. I like to sit quietly and read and the staff make sure this happens.' One relative told us 'I didn't look at another home, I just knew when I came here that this was the right one; I have been very impressed with the management and staff and have not had any cause to complain.'

We spoke to a district nurse that visits the home and she told us that the staff had a good awareness of health care issues and worked closely with the nursing services. The nurse, a regular visitor to the home, told us that staff had good awareness of the people's health and they were quick to refer any concerns, meaning that early diagnosis and treatment opportunities were taken.
