During a routine inspection

All people that lived at Rotherbank had a degree of confusion or dementia. Those that were able to told us that they were happy with the care and support provided and spoke highly of the staff. We observed that people were cared for in a sensitive and kind way.

People were provided with information about the service so that they could decide whether it would be appropriate for them.

People's needs and preferences were assessed and understood by an established staff team. Staff told us that they had time to get to know each person and this was endorsed by a visitor who also said that staff worked around the wishes of their mother.

People who were able to told us said that they felt safe at the home. Staff described an approachable management team who took action where necessary to comments made. Staff received an appropriate range of training to keep their skills and knowledge up to date.

During a routine inspection

We met with the registered owner and her son, who is the manager. They told me that there were currently nineteen people in the home, out of a total of twenty beds plus one respite bed, and were all aged over 60. All the people who used the service suffered from various degrees of dementia even though not all have been formally diagnosed as such.

We were able to speak to three people and they told us they were very happy with the service and the staff. We were unable to get a clear view from them regarding consent to care and treatment. However, two relatives we spoke to said they were always involved in care planning and were always asked to give consent. This was confirmed by the staff members we spoke to.

We looked at care plans and the care planning process was explained to us. Care plans were regularly reviewed and updated. We spoke to staff about their experiences of working in the home, and they said it was a very good working environment. We also looked at staff training and qualifications, and shift rotas.

We inspected the premises for any hazards or signs of disrepair, and everything appeared to be in good order. We also asked about how quality was monitored and feedback received. Most of this appeared to be done on an informal basis, albeit quite effectively.

We asked about notification of deaths to the CQC and discovered that the provider was failing to report all deaths of people who used the service.

During a routine inspection

When we arrived at the service we were met by the deputy manager as the provider who also managed the home was on holiday. The deputy manager assisted us throughout the inspection.

As many of the residents at Rotherbank Care Home had dementia we were accompanied on this visit by an 'expert by experience' so they could observe the care and talk to people to gain an understanding of the experiences of the residents. We have used the evidence from the 'expert by experience' for outcomes relating to respecting and involving the residents and for care and welfare.

In such a small home it was possible to talk to almost all the residents and each one echoed the words of the others by saying things like 'I am happy here,' 'the staff are lovely and kind,'and 'nothing is too much trouble for the girls.'

The residents, the relatives and the staff were all extremely positive about the care at this home. The residents said the care was 'excellent or wonderful' and the relatives agreed.

The staff made every effort to provide care and activities that suited each resident's needs and interests.

Two residents said they chose this home especially because they could bring their dogs with them. The home had just been awarded a prize for being 'pet friendly' and both residents attended an awards ceremony in London.
