During a routine inspection

We spoke with people who lived at Tanglewood Residential Home, the registered provider, the registered manager, care staff, a cook and two relatives.

Comments from people living at the home included, 'the food is beautiful', the staff are 'very helpful' and 'I like it here'. One person told us they felt the registered manager did not make decisions quickly enough and one person told us they felt if they complained then staff wouldn't speak to them.

We found, where people did have the capacity to consent, the provider acted in accordance with legal requirements.

There was an effective complaints system available.

People's needs were assessed, but care and treatment was not delivered in line with their individual care plan.

People who used the service were not protected from the risk of abuse, because the provider had not taken reasonable steps to identify the possibility of abuse and prevent abuse from happening.

People were not protected from the risk of infection because appropriate guidance had not been followed.

There were not enough qualified, skilled and experienced staff to meet people's needs.

The registered person did not have suitable arrangements in place to provide training to persons employed, to enable them to deliver care to an appropriate standard.

During an inspection looking at part of the service

When we visited Tanglewood, we met with eight people who lived there, either in the lounge, dining room, or in their own room. We met with two regular visitors to the home, the manager and three staff on duty.

People we met in the home told us they were very happy with the care and attention they received. One person said, 'They come quickly if you need them. It's very good in here, we have a quiz. I can have a bath whenever I like.' Another person said, 'We are wonderfully well looked after here.'

A visitor to the home said, 'There hasn't been anything to complain about ' if we have a question, we get an answer.' They said the staff were 'on the ball and very approachable.'

We found that staff worked carefully to meet the needs of people in the home, following guidance they were given.

We found that there was good provision of basic training but there had not been training for staff in how to provide care for people with the particular health and care problems of people living in the home. This meant that they may not know what to look for or how to support people as their condition developed.

At the previous inspection in April 2012 we found that record keeping was not good enough to ensure that people would be offered care appropriately. At this visit we found records were kept accurately and up to date, so that staff and management could monitor people's health and well being.

During an inspection in response to concerns

During our visit, we met with people in the lounge, the dining room and in their own rooms.

'They are good, really nice' said one person, talking about the staff. Another said 'The staff are capable ' pass on messages reliably, and the food is quite alright.'

One person had not been at Tanglewood for very long. They said, 'Staff are respectful. I have not had a problem having to wait when I need anything.'

A staff member who sometimes worked at night said that they found plenty of time to listen to people, and none had expressed concerns or said they were unhappy with the service.

Staff said they like to ask people what they would like, and not just tell them what is available. We saw that people had milk, tea and high energy drinks mid-morning and a choice of puddings at lunch time.

Some people mentioned that they would like a bath more than once a week. Staff thought that this would be possible. There had been problems with the bath hoist, but these were being dealt with.

A recently recruited member of staff told us, 'Everybody here has been so helpful', she said. 'I have never known anywhere so welcoming.'

We found that although staff levels at night had been reduced, there were sufficient staff to meet the needs of the people in the home. Arrangements were in place to increase staffing when people with higher care needs moved in or returned to the home.

We found that care records were not always accurately kept, securely stored, or easily found.

During an inspection in response to concerns

We carried out this review in response to concerning information. This focused on key themes, which highlight that the home may not:

' Have experienced and skilled care workers in sufficient numbers to meet the needs of people.

We carried out a review with an inspection to Tanglewood

on 8th February 2012and because of the concerns we looked the key outcomes covering respect and involvement, care and welfare, safeguarding people from abuse, safety availability and suitability of equipment, staffing and assessing and monitoring the quality of the service. The purpose of this review was to check compliance in these key outcome groups for people currently living in the home.

We looked at the records of three people in detail; and where possible we spoke to the individual and or their care worker. We observed other people being attended to whilst we were visiting. We also spoke to two visitors about their relation's experience of care and support at Tanglewood.

People we spoke to said that their needs were being met, with comments like 'the staff are all very kind and caring' and 'all the staff are qualified, only the seniors give out tablets'.

Families of people living in the home said that they feel very involved and 'always made welcome'. They also said 'we visit at different times, there's an open house with no restrictions. We can't fault the care she receives'.

Comments from a professional tell us that the "care is excellent at Tanglewood".

People are safe and their health and welfare needs met because staffing levels had been reviewed and increased.

Overall, we found that Tanglewood is a well run home that is constantly improving the quality of service that people experience.
