During an inspection in response to concerns

This responsive inspection was as a result of concerns that were reported to us from a whistle-blower who wished to remain anonymous. We found that their specific concerns about poor personal hygiene and the home smelling of urine were not substantiated by what we found during our inspection.

We spoke with two members of staff and 11 people who used the service. We sat and observed four others sat in the lounge. We also spoke with the manager and home owner.

People told us that care workers were "lovely". One comment made was 'they are all brilliant, they do everything for me'. One person said 'I am very happy here'.

We found that Doneraille had a clean environment which supported the delivery of safe care. We looked at four people's care plans and found that they contained up to date needs assessments. Daily care records showed that people had been supported with their personal hygiene and individual health needs.

During a routine inspection

We observed care practices, spoke to people, relatives and staff and looked at records to help us understand how people viewed the support they received at this service. This was because some people did not communicate clearly due to their dementia, and were therefore not able to fully tell us about their experiences.

Staff showed how well they knew each person's individual needs through their actions. We saw staff offered people choices, listened to people and were aware of people's body language where they did not communicate clearly. Staff were respectful and supported people patiently.

People told us that they liked it at this service . One person said ' I had been before to give my family a rest, but I liked it so much, I stayed'. Another person told us 'it's good to know there's someone about all the time'.

The service had policies and procedures in place that kept people safe. Staff were knowledgeable and confident about their safeguarding responsibilities and how they would respond to any concerns that may arise. People told us they felt safe. One person said 'The staff would know exactly what to do if I was worried about anything'.

Care records were clear to read and ensured people were at the centre of their care. They were regularly reviewed in order to remain up to date.

There were processes in place to monitor the quality of service being provided, and we saw that people and their relative's opinions were obtained through annual questionnaires.

During a routine inspection

On the day of our visit twenty four people were living at Doneraile. We spoke with four people. People told us that they thought the care provided was good, they were treated with dignity and their privacy was respected. One person told us the home was "tip-top." One person's relative told us, "I have been very happy with the care here, they have been very kind".

People had clear assessments of their needs and plans and strategies were in place to meet them. Information was communicated well within the staff team and people's care plans were reviewed regularly. People were supported to access healthcare services and the home worked well with health professionals. The home employed staff with a range of experience, skills and training to meet people's needs.

We saw staff interacting with people in a relaxed, friendly and respectful manner. Staff worked at the pace of each individual and encouraged independence. Social activities included sing a longs and quizzes. People who lived at Doneraile had friendships within the home.

People told us that they felt safe and that their possessions were secure. They knew how to complain if they had concerns. Staff had received training in safeguarding vulnerable adults and recognising abuse. The home had procedures in place for reporting and responding to any concerns.

The home had systems for quality assurance. We saw evidence that when improvements had been identified at Doneraile they were acted upon.
