Supported living services
This information is for supported living services only.
If you're planning to provide other services too (for example, home care), make sure you:
- send all the documents required for each service
- include relevant information for all your services in your documents.
Extra documents to send with your application
As well as the documents that everyone must send, you need to submit your:
- Business plan and financial forecast
- Evidence of legal occupancy
- Service user guides
- Staff training plan

Documents we might ask for
Our inspectors may need extra information to assess your application.
Do not send these documents with your application. Keep them ready in case we request them. If you cannot provide them when asked, your application may be delayed.

- List of risk assessments
- Person centred care planning policies
- Reportable incidents policy
- Risk management policy
- Sample care contract
- Sample care plan
- Staffing structure and rotas
