Important
From 5 May 2026, all oral health services will need to send additional documents with your applications. This change will help us review your applications more quickly.
This information is for oral health services only.
If you're planning to provide other services too, make sure you:
- send all the documents required for each service
- include relevant information for all your services in your documents.
Applications submitted before 5 May 2026
You only need to send the documents everyone must send with your application.
Applications submitted from 5 May 2026: extra documents to send with your application
As well as the documents that everyone must send, you need to send your:
- Critical examination and acceptance test reports
- Fire risk assessment
- Health and Safety Executive ionising radiations regulations registration
- Health and safety risk assessment
- Legionella risk assessment
- LOLER lift safety certificate (if applicable)
- Radiography risk assessment and Local rules document

Documents we might ask for
Our inspectors may need extra information to assess your application.
Do not send these documents with your application. Keep them ready in case we request them. If you cannot provide them when asked, your application may be delayed.
- Floor plan (sometimes called 'layout')
- Electrical installation certificate
- Emergency lighting completion certificate
- Gas safe certificate
- Fit and proper persons (directors) policy
- Medical emergencies and resuscitation policy and procedures
- Serious incident policy and procedures
We might also ask to see any relevant installation and calibration certificates for equipment, such as your:
- Decontamination equipment
- Amalgam separator and suction unit
- Air reprocessing unit or ventilation equipment (if installed)
- Pressure vessel testing for autoclave and compressor
Depending on your circumstances, we may ask for documents not listed here. We will tell you if we need anything else.
