Notifications for GP providers
All registered providers must notify us about certain changes, events and incidents that affect their service or the people who use it. This includes primary medical services.
You can find out more about statutory notifications by reading our guidance on meeting the regulations for providers and managers.
CQC Provider Portal
You can send us notifications online using our Provider Portal. It's the quickest, easiest way to notify us about events and changes, and it's available to all GP practices now.

We've developed your CQC accounts so that you can submit some notifications using our easy-to-use online forms.
Forms
The forms make clear the information you must submit but also ask for additional details to help us understand what has happened and how you responded to it.
If you provide this additional information, it will help us decide whether you handled the event correctly without having to visit or contact you.
Statutory notification: Changes to your statement of purpose – you can submit this notification using our Provider Portal.
You can use your Provider Portal account to notify us of most changes in this form. Only use the Word form below if you want to notify us of:
- changes to an organisation's directors or similar officers
- the appointment of a trustee in bankruptcy, a receiver, liquidator or provisional liquidator
- the sequestration of an individual's estate.
The forms are 'protected' Word documents. You can move from section to section by pressing your return, tab, page up, page down and arrow keys, or by using your mouse.
You can put crosses in check boxes by pressing your spacebar when they're selected or by clicking with your mouse.
