If you have experienced poor care, or know that poor care is being provided somewhere you can report it to us, anonymously if you wish. You can also tell us when you feel you have received good care.
You can look for services in our directory using the tabbed search box above. Once you have done this, you will see a list of services matching the postcode, place or name you entered.
If you click on the name of the service, on its profile you will see a tab with 'Please tell us your experience' on it - this is our online form for providing feedback on care.
