The Gainsborough practice is located in a purpose built medical centre which is shared with the branch surgery of another practice in Bracknell, Berkshire. The practice belongs to East Berkshire Clinical Commissioning Group (CCG). Approximately 9,980 patients are registered with the practice. The practice does not have a large number of patients in the older and younger age groups compared to the rest of the CCG and nationally. The practice does have a larger number of working age patients than the national average. National statistics show there is low deprivation and is predominantly a white British population, with 9% of patients deriving from black and other ethnic minority groups.

Two GP partners (both male, whole time equivalent (WTE) 2) operate the practice. There are five salaried GPs (4 female, 1 male, WTE 3). The practice is accredited to provide training for GPs, although there were no GP trainees in post at the time of the inspection.

The nursing team consists of a nurse practitioner (female, WTE 1) and two practice nurses (both female, WTE 1.65). There is a practice manager and a team of reception, administration and secretarial staff. Services are provided via a General Medical Services (GMS) contract.

The practice provides the following regulated activities: Diagnostic and screening procedures, Family planning, Maternity and midwifery services, Surgical procedures and Treatment of disease, disorder or injury.

The practice is part of a federation of Bracknell and Ascot GPs who provide extended hours appointments to all patients in the Bracknell and Ascot area from a dedicated surgery. GPs and nurses from across the area work for the federation to offer a variety of appointments on weekday evenings and Saturday and Sunday mornings.

The Gainsborough practice also became part of the Bracknell and District Primary Care Network in July 2019.

The practice has opted out of providing out of hours services to their patients. There are arrangements in place for services to be provided when the surgery is closed and these are displayed at the practice, in the practice information leaflet and on the website.

We carried out an announced focussed inspection of The Gainsborough Practice on 11 December 2019 to follow up on a breach of regulation from the previous comprehensive inspection in November 2018. This inspection followed our annual review of the information available to us. This inspection looked at the Effective key question only. We did not check whether this practice was providing safe, caring, responsive or well led services at this inspection because our monitoring of the practice indicated no significant change since the last inspection.

We based our judgement of the quality of care at this service on a combination of:

• what we found when we inspected

• information from our ongoing monitoring of data about services and

• information from the provider, patients, the public and other organisations.

We have rated this practice as good for providing effective services and good for all population groups.

During this inspection we found that:

  • The practice had reviewed their systems and processes for recalls and exception reporting and had achieved lower exception reporting outcomes when compared to previous data.
  • Learning and development was promoted and provided for all staff.
  • All staff at the practice were active in ensuring sustainable high quality care was provided to all patients.
  • Nurses were actively involved in clinical audit and quality improvement activity.

Whilst we found no breaches of regulations, the provider should:

  • Continue to engage with patients on the serious mental illness register to further improve access to care and reduce exception reporting.
