During an inspection looking at part of the service

The manager advised us that twenty people were living in the home. They told us that nine of the people living at the home required nursing care; one person was being nursed in bed and had a pressure sore; seven people required hoisting and four people required assistance to eat their meals.

We spent time talking with people in the home, several were aware of the home closing but raised no concerns. The activity organiser was in the home and we observed her encouraging people in colouring pictures and making collages, other people were watching television and several were reading newspapers.

We found that medication administration records had been completed appropriately.

We were unable to see evidence that a Criminal Record Bureau (CRB) check had been undertaken for one member of staff. The manager told us that missing documents were probably ready for filing at the other home owned by the provider.

Staff told us they had received training in safeguarding adults from abuse, but were unaware of the date. We requested to see training records, but these were not available as the manager had gone off duty.

We spoke with staff on duty who told us "there is not enough staff on duty for us to do all the personal care for the twenty people in the home. We need another member of staff".

Many of the records were not available because the manager had gone off duty or we were told, kept at the providers other home.

During an inspection looking at part of the service

Oxfordshire's Safeguarding Team have received seven safeguarding referrals had been made to them from sources outside the service.

The Commission had not been notified of these by the registered manager. None of the recent staff employed had experience of working with older people in a care setting.

The duty roster was not accurate as one of the staff rostered to work 2pm until 9pm, was also rostered to work at The Doris Watts Home from 2pm until 5pm.This reduced the staffing level in the afternoon to two staff and a nurse and no staff on duty at Clear Skies until after 4.30pm.

Three members of staff on duty had not been trained in moving and handling, although we were told that almost half of the people required hoisting.

We found confidential information in the office. We were also told that information kept on the office computer was not pass word protected and information kept was freely accessible.

During an inspection looking at part of the service

The inspection was carried out to follow up areas of non compliance found at a previous inspection in March 2012. We reviewed the outcomes in regard to the compliance action and two improvement actions found at this last inspection visit. We also looked at the outcome relating to the care and welfare of people using the service to check that people received the support they required. People told us that they were given the support they needed which included seeing their GP and other healthcare professionals.There was a mixed response by people about the activities and social life at the home. Some said the activities available suited their needs, others said that there wasn't much to do, one person said it was 'boring' and that there was very little opportunity to go out of the home.

We found during the visit that there was the potential for people to be put at risk through practices for the safe storage of chemicals, food safety and the storage of disused furniture in outdoor spaces. These concerns were rectified during the inspection visit but we were unable to test that this compliance was sustained. We identified that there was a moderate risk to people as appropriate pre employment checks were not carried out on the suitability of staff. Criminal Records Bureau (CRB) and Independent Safeguarding Authority (ISA) list checks had not been received for two members of staff before they had commenced working in the home.

During an inspection in response to concerns

People told us that they enjoyed living in the home. People told us that staff were helpful and listened and acted upon their choices of how they wanted to live.

People told us that they liked living in the home and they received the help they needed. Staff were described as 'lovely' and they 'can't do enough for you.'

People told us they were able to go to bed and get up when they wanted to. One person told us that he liked to have a bath every morning and staff supported him to do this.

One person told us that they would have liked the opportunity to be involved in more activities. Another told us that some of the activities did not interest them so they stayed in their room. People told us that they went out of the home for shopping, visiting friends and pub visits.

People told us that they had a choice of food and drink throughout the day. For breakfast one person told us he had egg and bacon everyday. We were told that there were always two choices of cooked food at lunchtime.
