All Inspections

5 December 2023

During a routine inspection

We carried out this announced comprehensive inspection on 5 December 2023 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions.

We planned the inspection to check whether the registered practice was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations.

The inspection was led by a Care Quality Commission (CQC) inspector who was supported by a specialist dental advisor.

To get to the heart of patients’ experiences of care and treatment, we always ask the following 5 questions:

  • Is it safe?
  • Is it effective?
  • Is it caring?
  • Is it responsive to people’s needs?
  • Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

  • The dental clinic appeared clean and well-maintained.
  • The practice had infection control procedures which reflected published guidance; however, these were not consistently followed.
  • Staff knew how to deal with medical emergencies. Appropriate medicines and life-saving equipment were available.
  • The practice had systems to manage risks for patients, staff, equipment and the premises. However, systems to ensure facilities were being maintained needed improving.
  • Safeguarding processes were in place and staff knew their responsibilities for safeguarding vulnerable adults and children.
  • The practice had staff recruitment procedures which reflected current legislation.
  • Clinical staff provided patients’ care and treatment in line with current guidelines.
  • Patients were treated with dignity and respect. Staff took care to protect patients’ privacy and personal information.
  • Staff provided preventive care and supported patients to ensure better oral health.
  • The appointment system worked efficiently to respond to patients’ needs.
  • The frequency of appointments was agreed between the dentist and the patient, giving due regard to National Institute of Health and Care Excellence (NICE) guidelines.
  • There was effective leadership and a culture of continuous improvement.
  • Staff felt involved, supported and worked as a team.
  • Staff and patients were asked for feedback about the services provided.
  • Complaints were dealt with positively and efficiently.
  • The practice had information governance arrangements.

Background

Frederick Street Family Dental Practice is in Sunderland and provides NHS and private dental care and treatment for adults and children.

The practice is accessible for people who use wheelchairs and those with pushchairs. The practice is located close to local transport routes and car parking spaces are available near the practice. The practice has made reasonable adjustments to support patients with access requirements.

The dental team includes 3 dentists, 3 dental nurses, a practice manager, and an administrator. The practice has 3 treatment rooms.

During the inspection we spoke with 3 dentists, 1 dental nurse and the practice manager. We looked at practice policies, procedures and other records to assess how the service is managed.

The practice is open:

Monday from 9am – 6pm

Tuesday from 9am – 5:30pm

Wednesday from 9am – 5pm

Thursday from 9am – 12:30pm

Friday from 9am – 5pm

There were areas where the provider could make improvements. They should:

  • Improve the practice’s infection control procedures and protocols taking into account the guidelines issued by the Department of Health in the Health Technical Memorandum 01-05: Decontamination in primary care dental practices and having regard to The Health and Social Care Act 2008: ‘Code of Practice about the prevention and control of infections and related guidance’.
  • Take action to implement any recommendations in the practice's fire safety risk assessment and ensure ongoing fire safety management is effective.
  • Improve the practice's systems for checking and monitoring of facilities taking into account relevant guidance and ensure that all equipment is well maintained.

During a routine inspection

There were suitable arrangements for obtaining peoples consent for the care and the treatment provided to them. People told us they were happy with the service provided. One person said 'I like the way they always give me options and I know about prices because of all the information displayed in the surgery'. Another person told us, "It's a really good practice. I've been coming here for some time and all of my family come including my daughter and her two children."

People experience effective, safe, appropriate care and support that meets their needs. This was confirmed by people we spoke to and by the records which were kept. One person said 'We all like coming to the surgery and it is always really clean and well organised'. An individual told us that the dentist always explained the options available. They said 'he is really good at telling me in plain language what I need done'.

The staff we spoke with knew how to carry out their role and continued to maintain their skills and competencies to make sure they remained safe to practice. One person told us that they are sent reminders when they need to have their routine appointments for check ups and that 'this helps us to make sure that all of the family keep up to date with our dental care'.

We found that people were protected from the risk of infection because appropriate guidance was followed.
