Standard version

This version should allow you to cut and paste into the AQAA form and use your own word processing software. If you use the unlocked AQAA form, make sure you haven't deleted or altered any questions before sending it back to us.

Read the guidance

The annual quality assurance assessment is in two parts:

  1. A self-assessment that asks you to tell us how well you think you are meeting the needs of the people who use your service. (We will be using the current outcome groups contained in the national minimum standards for your type of service.)
  2. A data set that asks you to give us some basic facts and figures about your service.

What to do with the form

We will contact you when it's time for you to send us your filled in annual quality assurance assessment. You will have four weeks from when we contact you to return the form.
