The application form
Once you've received a CQC-countersigned DBS certificate, you can complete the provider application. Please don’t submit your application until you've received a green copy of your DBS and the relevant registered managers have completed their applications to continue registration as a manager under a new provider.
We have produced a guidance document offering help with the application form. Please note, however, that you can get help on individual sections by clicking the 'See Guidance' links within the form itself.
The application form contains space to include details of one location, one nominated individual and two partners. If you need to include details of more than this, you will need to complete the relevant section below for each additional one.
