What types of service do you manage?
The form you need to use will depend on the types of service you manage. There are two forms below. One is for managers of GPs and other primary medical services. The other is for managers of other health and care service managers.
GPs and primary medical services
Who is this form for?
This form is for use by managers of primary medical services. It should only be used by registered managers who have completed the registration process with us and now want to make changes to any conditions placed upon them.
When this form might be used
A registered manager responsible for three regulated activities at a GP practice has a compliance condition placed upon their registration requiring them to complete training in the safe handling of medicines by the end of a three-month period.
The training has now been completed and the manager wishes to have the condition lifted. In this situation, or equivalent ones, this is the form that will need to be used.
